No waiting list or make up dates

9:00AM to 2:00PM on
OCTOBER 21st & 28th
NOVEMBER 4th & 11th

You MUST have:

1. Photo I.D.
2. Proof of income for each person living in the home
3. S.S. cards for each person in the home
4. Two proofs of "CURRENT" address
